Fill out page 3 & 4 of this form and send with your application.

You DO NOT need to see a doctor to get the Novice permit, only a full TT license, read the fine print & ENJOY!
Oh, and be sure to send the money for the license.

And, remember, if you upgrade your novice permit to a full license within 90 days, you don't have to pay the $35 again to get the TT comp license.
